This has everything to do with the lack of standardized terminology in use at various institutions. In order for data to be understood and compared, data must be defined and used in the same manner. This standardization is critical in assessment, clinical decision making, and public health surveillance. It is also critical in order to follow patients across sites of care.

Data mining helps in process improvements and quality assurance. It also is essential in the identification of best practices and development of improved support systems. Unless data are consistently coded, this process become much more difficult.
