Many of our concepts link together with use of technology. For example, meaningful Use to National Safety goals. It is important to start linking all of these concepts together for the focus on quality organizational and patient outcomes. The US Department of Health and Human Services (USDHHS) has a great handout that links the stages of meaningful use to patient safety goals to patient outcomes and practical application.

For example, one focus is on advanced care planning and the presence of an advanced directive in the electronic medical record. According to the USDHHS, “Core for Eligible Hospitals, introduce as Menu for Eligible Professionals • Record whether a patient 65 years old or older has an advance directive • Threshold: Medium • Certification Criteria: CEHRT has the functionality to store the document in the record and / or include more information about the document (e.g., link to document or instructions regarding where to find the document or where to find more information about it” (n.d., “Updated stage 3 objective).

According to Skrocki, the electronic medical record has created unique challenges in the sharing of information. This has everything to do with the lack of standardized terminology in use at various institutions. In order for data to be understood and compared, data must be defined and used in the same manner. This standardization is critical in assessment, clinical decision making, and public health surveillance. It is also critical in order to follow patients across sites of care.

Data mining helps in process improvements and quality assurance. It also is essential in the identification of best practices and development of improved support systems. Unless data are consistently coded, this process become much more difficult.
