The assessment for this module will consist of up to 2000 word assignment on the following topic.
Develop a plan to promote individual health and wellness supported by appropriate theoretical perspectives from the scenario.
You will need to use the following outline to structure you plan:
Introduction
Your introduction needs to answer the following questions:
1. What? Give a brief outline of what the plan is about. This is what is given in the assignment brief above.
2. How? Give a brief guide of the structure of the plan, that is, a brief chronological outline of the plan. You need to refer to the detailed guidance given below.
3. Why? Briefly give a rationale for your plan. In other words explain why promoting the health of the chosen individual is important. This could be relate to module learning outcomes; personal
development plans; policy; research etc. You will need a brief literature overview of the subject area.
Discussion
1. Identify an individual who can benefit from health promotion from your field of practice. Give a clear outline of the individual, focussing on the health needs that necessitate the need for
health promotion.
2. Identify and discuss the bio-psychosocial factors that contribute to the health and wellbeing of the individual.
3. Clearly outline the activities that need to be undertaken in order to promote the health of the individual [you could include a detailed plan in an appendix]. You need to justify or give reasons
for each of the actions that need to be undertaken.
4. Identify, discuss and evaluate the appropriateness of different approaches and strategies that can be utilised to promote the health of the individual.
Conclusion
1. Summary of key learning points identified within the plan.
2. Recommendations as to what needs to happen next. This could be your own learning development needs; policy or research developments etc.

Molly is 35 years old and she is married to James. They have two children Eric who is 8 years old and Sebastian who is 3 years old. James works as a long distance lorry driver and spends long
periods abroad. Molly was diagnosed with type 2 diabetes two years ago, and manages her condition well. Molly has just discovered she is 8 weeks pregnant. She hopes it will be a girl this time but
is worried she will suffer from post natal depression as she felt very low following Sebastian’s birth. She has a part time job at a local supermarket fitting her shifts around school and nursery.
Molly likes to go to the gym but finds it difficult to find time to get there.
James (Molly’s husband) is 37 year old and works as a lorry driver. He has to travel long distance and currently spends more time abroad than at home. James is pleased that Molly is expecting
another baby. Recently he has started looking for work closer to home so he can offer more support to Molly. Unfortunately he has been unable to find a suitable job close to home but continues to
look for one. Molly has noticed that James gets worried. She has also noticed that he is low in mood, irritable and loses his temper easily. He has started to smoke again, a habit he stopped 4
years earlier. His alcohol intake has also increased.
Molly’s parents have been very supportive but her father has just been diagnosed with dementia. Molly’s father (David) was diagnosed with Alzheimer’s about 18 months ago. Most of the time he
manages well but he relies on his wife to remind him to take his medication. Recently he got lost whilst out shopping without his wife, a friend saw him and helped him to get home. He is now
reluctant to go out alone in case he gets lost.
Molly and James have two children Eric who is 8 years old and Sebastian who is 3 years old. Towards the end of Eric’s primary school years, he was found to have a learning difficulty and to be on
the lower end of the autistic spectrum. He attends a mainstream school with additional support in the school. He uses limited speech to communicate therefore gets frustrated when people fail to
understand him and at times displays behaviour that is difficult to manage. There were episodes when Eric was picked on by other children in the school. Eric has limited social contacts therefore
relies on his immediate family for social support. Eric is severely overweight as he tends to have a fixation with food.
Sebastian attends the nursery attached to Eric’s school. His nursery teacher has noticed that he is not interacting well with his peers due to his mild hearing loss (caused by Otitis Media). He has
no other long term problems but has recently had several episodes of earache and tonsillitis. He has had an appointment with an ENT specialist. The ENT specialist said that Sebastian does not
require surgery at the moment but advised Molly to discuss with her Health Visitor how she can help Sebastian to reach his full potential developmentally.
