In the previous chapters we discussed the stages of this implementation using The HSE Model for the change, the main reason for this change were improve patient safety the objectives for this change were the following:
• By 30th March 2016, the physicians will meet (hit) 95% compliance rate against new system guidelines.
• By 30th March 2016, the total number of dispensing near miss incidents will be decreased by 99%.
• Pharmacy visit experience’ patient satisfaction will be maintained 90% and above by 30th March 2016. Or, patient stratification post pharmacy visit will be maintained 90% or above by 30th March 2016.
• By 30th March 2016, all pharmacists will be able to demonstrate 100% compliance rate to new system protocol.
