Choose a health care-related topic on which to do research using a survey. I suggest that you choose something that interests you.

2. Write the hypothesis for your study.

1. Survey at least 25 individuals documenting their answers to your ten (10) questions.

2. Chart the answers to each question obtained in a Table created in Microsoft Word.

3. Write a statement on this document as to your method of surveying these people (ie. in person, via Survey Monkey, via email)

3. Submit the Table .
