Patient Info:
About the client
The client was a 65 years old men from Michigan. Living in Florida since 1986. He was an auto mechanic, He is retired now. He has two children, a son who is living in Florida and a daughter who lives in Wisconsin. He loves race car. He has been Disable since 1978. He used a scooter to move around. He has a college level one term only. He is a smoker.
Client condition when he was admitted
1. Urine retention
2. Atherosclerotic heart disease
3. Hyperlipidemia Unspecified,
4. Physical disability
5. Skin rash
There was no changes since he was admitted, all of the conditions are under control.
Client medications
1. Cetaphil restoraderm lotion (2 times a day after shower).
2. Econazole Nitrate Cream (2 times a day)
3. Ciclopirox 0.77% cream
4. Atorvastatin 10 mg tablets (1 table by mouth at bed time)
5. Senexon S tablet (1-2 tablet by mouth as needed)
6. Aspirin 81 mg (1 tablet daily)
Instruction to write the paper:
Please write a brief (3 pages) reflective paper that addresses the following.
1. The resident
i. Describe him or her so the reader gets a mental picture of the patient.
ii. Are there factors that are affecting the patient’s willingness to follow their medication regime (Patient stated that he surely want to take his medication as prescribed because he does not want to have another heart attack.)
iii. Provide a review of the history and physical and major illnesses/diagnoses (see client info file)
iv. Diet, code status, allergies (Client is on regular diet, He has no known allergy)
v. Address any issues of compliance and how you, if you were the nurse, might resolve those issues with the patient(there was none, client was very compliant)
vi. The patient’s level of understanding of the medications including your evaluation of whether the patient’s degree of understanding is sufficient for safe use of this medication – what might you do if you were the patient’s nurse about this. (Patient was very smart. He has a faire knowledge of all his medications. He knows all about cholesterol, all the risks for smoking, and what all the medications are used for. He only asked us to teach him about the side effects of those medications)
