Instructions from the instructor:
To begin, before you can priorities your care and implement a specific intervention you (as in all of your clinical practice as a registered nurse) will first need to perform an assessment that is relevant, this will add strength to your chosen priority by demonstrating to the reader why it is that you’ve chosen this priority. You must then explain the priority, with relevant reference to the chosen client. This essentially, is the first three components of the clinical reasoning cycle, consider the patient, collect cues and information and process this information and essentially will end with you identifying the priority (known as identifying problems/issues). You are then required to establish goals relevant to the patient and in doing so you must justify why have you chosen this priority- this is the taking action component of the cycle. You must support your chosen priority with the most relevant and up to date evidence and then describe how you will assess the success of your suggested intervention or in other words you will be required to reflect on the practice you have just suggested.

The number of priorities in the first visit is entirely up to you; however remember there is a 1600 +/- 10% word limit for the paper.
You are then required to, using the same process as above, identify a further two priorities for your follow up visit.

Firstly, according to the marking criteria, it mentions “appropriate for the target audience”, I want to know who is the target audience, is it the patient in the scenario, or the teacher who reads the essay? use the first person or the third person to write this essay?
Secondly, “prioritise and provide a rationale for the activities you will undertake as part of this initial visit”, what are included in the” activities”, is it only the assessments for the patient with related the current issue?
Thirdly, should we discuss the pathophysiology, treatment, and pharmacology for the current issue?

Response from the instructor for above question:
In relation to the first part of your question, the marking rubric states ‘The language is sophisticated, and appropriate for the target audience, and suits the purpose for which the essay is intended’ this means that the language you use to write your assignment should be at the level of a third year nursing student demonstrating a depth of understanding that reflects your expected level of knowledge. Remember that the target audience could be, for argument sake, health care professionals including registered nurses and therefore you should use language that is appropriate.
In response to the second part of your question, I would like you to consider how it is that you intend to identify your care priorities without first doing an assessment? Is this possible? Think about how you come up with care priorities in your nursing placements? Do we simply pull our priorities from thin air or is there an assessment that takes place prior to us making this judgement.
Additionally, you are required to suggest interventions applying the clinical reasoning to do so to enable you to justify why you have identified these as a priority.
The third part of your question depends entirely on what your priorities actually are, and as these will be specific to each individual student there is no right or wrong answer here, some priorities may include a brief discussion of pathophysiology, treatment and pharmacology if this is an identified priority, however, remember not to get bulked down with this and relate it only to your priority, the purpose of the paper is to demonstrate your ability to prioritise care and provide a rationale for that that care using evidence to support it not solely to demonstrate your knowledge of pathophysiology and pharamacology.
