Order Description
Project title: Anticoagulants safety in clinical practice.
It is an audit of 232 incident reports of anticoagulants-associated medication errors in a hospital, for the period of 3 years (April 2014 to March 2017).
I have uploaded my proposal for this dissertation so you can have a look at the aims/objectives to have an idea. The data of incidents are supplied into 3 Excel
spreadsheets. I have analyzed them into 2 Excel files (according to the division of the hospital, severity of the incident (actual harm), error type, RRN…etc.
I have uploaded a similar dissertation from the last year (data is different), so please have a look at it. Please do not copy it as it is as it is provided by my
supervisor. Please cover similar or same aspects/subheadings in the different parts of the dissertation.

Background/Lit.Review: 2500 words
I have written 1500 words of the literature review in the proposal, please use it, amend it or change some parts according to your expertise to cover more and relevant
aspects. (have a look at the dissertation provided). Please provide figures/Tables where appropriate. The word count for the background/Lit. review should be 2500
words not less.
Aims/Objectives: already available in the proposal.
Methodology: 1200 to 1500 depends on you
Have a look at the methodology in the proposal.
My supervisor focuses a lot on the methods section. Excel is used for analysis, not SPSS.
– You may start with audits in general, identification of medication errors…etc.
– Have a look at the analysed data Excel files (1&2) & 3, to identify the different variables, how the data are anaysed quantitatively and qualitatively.
– Anonymization of data (the data was given by a member of staff in the Trust and totally anonmised, no names … see proposal).

Results: 1000-1300 words:
Have a look at the dissertation provided if you want, or do it per your style. Please provide charts, columns…etc for description.

Discussion/Recommendations/Limitations/Conclusion
The rest of the word count > 3500 words. Please Remember, Discussion is the heart of the dissertation.
